An identification code (taxpayer registration number) is a unique number assigned to every individual in the State Register. It is required for Ukrainian citizens and foreign nationals to take up employment, set up a sole trader business, purchase property, open bank accounts, receive income and carry out many other procedures. To obtain an identification code, you must apply to the State Tax Service with a specific set of documents.

1. For Ukrainian citizens (of legal age)
A document certifying the identity of a Ukrainian citizen:
Additionally (to confirm place of registration if no details are provided in the passport):
- Ukrainian passport (ID card or booklet-style passport).
- Ukrainian passport for travel abroad (only for those who are permanently resident abroad and have a stamp confirming permanent consular registration).
- Temporary certificate of Ukrainian citizenship.

- An extract from the local authority register (available from a Centre for Administrative Services or online via the Diya portal).
- A certificate confirming registration as an internally displaced person.
2. For foreign nationals and stateless persons

A notarised translation into Ukrainian of the identity document (the original will be returned upon presentation) + a copy of the translation. Exception: persons holding permanent or temporary residence permits in Ukraine need only provide a copy of their permit.
Where to apply:
- The foreign national’s identity document (foreign passport or other document recognised by Ukraine).
- Permanent residence permit (for those holding a permanent residence permit).
- Temporary residence permit (for those holding a temporary residence permit).
- Refugee certificate.
- Certificate of a person in need of subsidiary protection.
Required for most foreign nationals:
A notarised translation into Ukrainian of the identity document (the original will be returned upon presentation) + a copy of the translation. Exception: persons holding permanent or temporary residence permits in Ukraine need only provide a copy of their permit.
Where to apply:
- Usually at the main offices of the State Tax Service (Kyiv, regional offices).
- Foreign nationals with residence permits may apply at their local tax office.
3. For children (minors)
For children under 14 years of age (minors):

For children aged 14–18 who have not yet obtained a passport: the same documents as for minors. If the child already has a passport, they apply as an adult.
- The child’s birth certificate.
- A document proving the identity of one of the parents (or legal representative: adoptive parent, guardian or carer) – a Ukrainian passport.
- A completed registration form No. 1DR (documents to be submitted by the father or mother).
Additionally
- If the birth certificate was issued abroad by another country — a notarised translation + a copy.
- If one of the parents was a foreign national at the time of the child’s birth, or if the child was born abroad — a certificate from the consulate confirming the child’s registration as a Ukrainian citizen.
- Proof of place of registration (if required).
For children aged 14–18 who have not yet obtained a passport: the same documents as for minors. If the child already has a passport, they apply as an adult.
4. Registration by power of attorney (through a representative)
This is the most convenient option for foreign nationals, people living abroad and those who are unable to visit the tax office in person.

- A power of attorney, certified by a notary (for registration in the State Register). If the power of attorney was issued abroad, it must bear an apostille and be accompanied by a notarised translation into Ukrainian.
- A document proving the representative’s identity.
- A document proving the principal’s identity (original or a clear photocopy).
- All documents required for the relevant category of person (passport, translation, etc.).
General requirements for documents
- All documents must be in Ukrainian or accompanied by a notarised translation.
- No corrections are permitted.
- If the passport details are in Latin characters, the form must show the Ukrainian version followed by the Latin version separated by a slash.
- Submission: in person, through a representative, by registered post or online.
